Step 1.  Download Form

Step 2.  Fill in your information and then print it out.
If you do not have Adobe reader you can download it for free from the following website: http://get.adobe.com/reader/.If you already have an Adobe Reader click on the file, complete information and Print.

Step 3. Submit Form.  Once you have completed and printed your form, sign the application and send the completed application to us either by fax, e-mail (scan both pages with your signature and e-mail it to us), mail or drop it off at your local North American Lumber store.
